Women with High BMI Produce More Follicles and Require Less Gonadotropins During IVF

Obesity adversely affects the reproductive health of a woman as it is known to cause anovulation, menstrual irregularities, and infertility. However, the findings of various studies on the influence of BMI on controlled ovarian hyperstimulation factors, such as quantity of gonadotropin and days of stimulation required, oocyte quality/quantity, maximum level of estradiol, and success of pregnancy, are inconsistent. A recent prospective study reports that women with elevated BMI produce more follicles, and require fewer days of stimulation and gonadotropin during IVF treatment, although no significant effects on pregnancy outcome rates were reported. NEWS: Semen Washing Does Not Eliminate Risk of HPV Infection

Conventional sperm recovery procedures are hypothesized to depend on the migration of motile sperms away from washed epithelial cells and immotile sperms, thus leading to the elimination of infected epithelial cells and enabling virus-free sperm preparations. Although this seems biologically plausible, a recent cross-sectional clinical study highlights that conventional sperm selection techniques may not entirely remove the risk of HPV infection. NEWS: ESHRE Defines Poor Response to Ovarian Stimulation

The lack of clear consensus on the definition of poor ovarian response (POR) in ART has made it difficult to compare results of various studies conducted to improve ovarian response; in turn hindering the identification of poor responders and selection of an optimal ovarian stimulation strategy. In a first realistic endeavor, the European Society of Human Reproduction and Embryology (ESHRE) working group proposed a standardized, simple, and reproducible definition for POR. FSH Administration during hCG Trigger may Improve Oocyte Developmental Competence in IVF Cycles

The increase in the success rates of ART over the past few decades has been mainly attributed to improved laboratory conditions and ovarian stimulation optimization, with less attention directed towards strategies for enhancing final oocyte maturation. A recent study, touted to be the first randomized double blind placebo-controlled clinical trial to alter the ovulation trigger to enhance the developmental competence of oocytes in IVF cycles, has reported a statistically significant improvement in fertilization after a follicle stimulating hormone (FSH) bolus administration at the time of human chorionic gonadotropin (hCG) trigger, compared to placebo, in IVF cycles. Transabdominal Follicular Aspiration Effective for Oocyte Retrieval from Transvaginally Inaccessible Ovaries

A new retrospective case-control study has reported that oocyte retrieval via transabdominal ultrasound-guided follicular aspiration is safe and effective in IVF patients with substantial ovarian displacement, i.e. in cases where ovaries are inaccessible transvaginally. The study reported that the results were comparable to cycles where the oocytes were obtained by transvaginal aspiration in women with normally located ovaries. Continue Reading

Sperm DNA Damage Measurement by Alkaline Comet Assay May be Useful in Male Infertility

Low chromatin packaging quality in morphologically normal spermatozoa is one of the important factors that lower the fertilization capability of the sperm. Therefore, evaluating DNA integrity is essential to determine actual sperm fertility potential. A recent prospective study published in Fertility and Sterility provides new evidence that DNA integrity analysis using alkaline comet assay may be beneficial in diagnosing male infertility and predicting IVF outcomes.
